Send your models or impressions
We are happy to accept traditional impressions and poured plaster models. For physical case submissions, please include our lab prescription slip and use our mailing labels so cases are received and processed efficiently.
Scans + Bands (+ Bites …oh my!!)
Submit digital scan + send bands (or bites)
For cases that include both digital scans + physical components, please submit the digital scan through EasyRx first. Ship any physical items and label them with the patient’s name, doctor’s name, practice name, EasyRx prescription ID, and a contact phone number so we can accurately match them to the correct case. Bands may be shipped using our mailing labels or sent directly to our PO address:
